• Incoming Freshmen

  Below are the items that need to be completed and how:   

  1. Conditional Questions - completed directly in Magnus - answers to these questions may prompt additional forms to be completed by you and your healthcare provider
  2. Vital Health Record - completed directly in Magnus
  3. Immunization Form - form is completed by healthcare provider, then uploaded into Magnus
  4. Physical Exam and Health History Form - 
     1. Physical Exam Form is completed by healthcare provider
     2. Health History Form is self-completed, signed/dated by student and parent/guardian
     3. BOTH forms are uploaded together into Magnus
  5. Emergency Consent and Permission for Over-the-Counter Medication Form - completed directly in Magnus
  6. TB Risk Assessment - completed directly in Magnus

• Transfer Students

  Below are the items that need to be completed and how:

  1. Conditional Questions - completed directly in Magnus - answers to these questions may prompt additional forms to be completed by you and your healthcare provider

  3. Immunization Form - form is completed by healthcare provider, then emailed to school nurse
  4. Physical Exam and Health History Form - 
     1. Physical Exam Form is completed by healthcare provider
     2. Health History Form is self-completed, signed/dated by student and parent/guardian
     3. Email both forms to school nurse
  5. Emergency Consent and Permission for Over-the-Counter Medication Form - completed directly in Magnus
  6. TB Risk Assessment - form can be self-completed, then emailed to school nurses

• Trying out for sports at Padua Academy?

  We follow the protocols set forth by DIAA (Delaware Interscholastic Athletic Association).  As a result, we require the sports forms used by DIAA to be used by our students.  Be sure to print these forms out to take with you prior to going to see your doctor.  These forms can be found on Magnus, printed, completed and then uploaded back into Magnus.  

  
  

  Important information regarding dates: Forms must be dated on April 1st or later for the following year.  Example: for the 2023-2024 school year all forms must be dated 4/1/2023 or after.

  
  

  The required sports forms are:

  • Physical Exam - completed by healthcare provider and uploaded to Magnus
  • Health History Form -  self-completed, signed/dated by student and parent/guardian (physical signatures required)
  • School Athlete Medical Card - self-completed, Section 1 & 2 must be completed in full.  Use N/A in Section 2 if needed.  Section 3 - both student and parent/guardian must sign and date. (physical signatures required)
  • DIAA Pre-Participation Physical Evaluation/Consent Form - #1 must be signed and dated by both student and parent/guardian; #2-5 must be signed and dated by parent/guardian.   (physical signatures required)
