Student Illness/Absence:
- If your daughter is not feeling well, please keep her home
- If your daughter has a fever - she needs to stay home until she is fever free for 24 hours without fever reducing medication
- Any absence from school of 3 days or longer requires a doctor’s note upon return
- Any absence from school for hospitalization of any nature, requires a doctor’s note upon return
- If you have any questions, please do not hesitate to contact the nurse.
What is Magnus?
Magnus is the electronic health records program we use here at Padua Academy. All of the health records are entered in Magnus, either directly in the program or uploaded to it. The link for Magnus can be found in Veracross. You will have access to Magnus once your child is officially enrolled.

Incoming Freshmen

- Immunization Form - form is completed by healthcare provider, then uploaded into Magnus

- Emergency Consent and Permission for Over-the-Counter Medication Form - completed directly in Magnus
- TB Risk Assessment - completed directly in Magnus

Athletes: Additional Forms To Be Completed
If your child plans to participate in sports, they must complete the additional forms included in the DIAA 2026–2027 packet below.
Padua Academy follows the requirements of the Delaware Interscholastic Athletic Association (DIAA), so all students must use DIAA-approved forms. Please print these forms from Magnus or from the button below and bring them to your doctor’s appointment. Once completed, upload them back into Magnus.
Important: All forms must be dated April 1, 2026 or later for the 2026–2027 school year.
Required DIAA Forms:
- Pre-Participation Consent Form – Completed, signed, and dated by both the student and parent
- School Athlete Medical Card
- Sections 1 & 2: Completed in full, signed, and dated by student and parent (use N/A if needed)
- Section 3: Completed by a physician, including clearance, date, and signature
All completed 2026-2027 forms must be uploaded to Magnus. NOTE: the forms have changed, the new 2026-2027 forms need to be used; old forms will be rejected.
